What is collected and why
Only the name, phone number, email address and general details you enter in the appointment form, on the basis of the consent you give when you submit it. It is used to return your call and schedule a visit, nothing else. Please do not send medical details through the form; bring those to your appointment.
No tracking, no selling
This site sets no advertising or analytics cookies and does not profile visitors. Your information is never sold, rented or shared for marketing, and no health information is used for marketing of any kind. The location map is embedded from Google Maps, which may receive your IP address when it loads; the directions button opens Google Maps in a new tab. The video on this page is hosted by YouTube and only contacts YouTube once you press play, using its no-cookie player.
Health information
This website collects no health information. Nothing you enter here becomes part of a medical record, and the form is an ordinary web form, not a secure or encrypted channel for health details. Anything you share during your care is treated as confidential under Florida law, including section 456.057, Florida Statutes, and under HIPAA where it applies to this practice. A written notice of privacy practices is available at the clinic on request, and you may request a copy of your treatment records under Florida law. Nothing that identifies you, including your name, photograph, condition or comments, is published anywhere without your signed written authorization, which you may revoke in writing at any time. If information about you were ever exposed, you would be notified as required by the Florida Information Protection Act.
How long it is kept
Appointment requests are deleted once the request has been resolved and no visit follows. Records for patients who are treated are kept only as long as Florida law requires for clinical records.
Your rights
You may ask for a copy of what is held about you, have it corrected or erased, restrict or object to its use, or withdraw your consent at any time. Withdrawing does not affect care already given. Call 850-982-4734 or ask Scott in person and he will act on your request. If you are in the EU, EEA or UK, you also have the right to complain to your national data protection authority.
